Abstract
The athletic trainers at Wyoming High School do not have a system in which they upload pre-participation documents. These forms are an OHSAA1 requirement and helps with medical history. The way they receive athletes’ physicals is not ideal because these papers can get lost, a violation of HIPPA and results in athletes not being able to participate in their sport until the athletic trainer has it. The aim of this study was to reduce the number of physicals on the athletic trainers’ desk by 70% when she walks in every day. The two athletic trainers and I at Wyoming designed an instruction sheet for parents and guardians on how to upload their child’s physical directly to final forms. The outcome of this study was a 60% change to uploading through final forms. Although this was not our goal, it was extremely close. Although this kind of worked, a better time to have done this study would have been before fall sports began. This would increase the number of participants and see if the change would increase.
